The label hour for 50 mg
Swallow 50 mg. Fasted if the evening can spare it.
Earliest RigiScan responders in the fasted onset study.
Usual clinical onset people mean by 'it started'.
Median Tmax when fasted. The label hour.
Stimulated response still seen in the window study.
Terminal half-life of parent and N-desmethyl metabolite.
For most patients the recommended dose is 50 mg as needed, about 1 hour before sexual activity. Frequency is once per day.
The same label permits 30 minutes to 4 hours. One hour is still the planning number because median Tmax when fasted is 60 minutes. The range is 30 to 120 minutes.
RigiScan work in fasted men put median onset of 60% rigidity at 25 minutes, range 12 to 37, in those who responded. A separate study still saw a stimulated response 4 to 5 hours after the dose.
Swallowing 50 mg at the last minute only works if that man is on the fast tail of Tmax. Many men are not. Plan the hour. Do not test the 12-minute edge on a first night.
Raising the tablet to 100 mg does not fix a late swallow. The 100 mg step is for effect after a fair 50 mg trial, or the 25 mg step is for tolerability. Neither step is a timing patch.
Stimulation is required. A 50 mg tablet on a quiet sofa does nothing visible. The drug protects cGMP. It does not start desire.
Who starts at 25 mg instead
| Situation | Dose note | Timing note |
|---|---|---|
| Most men | 50 mg as needed | About 1 hour; 30 min to 4 h allowed |
| High-fat meal | Same milligrams, lower peak | Expect about 60 min extra delay |
| >65 y, liver, CrCl <30 | Consider 25 mg start | Same clock |
| Alpha-blocker on board | Start 25 mg if stable | Do not stack surprises |
| Ritonavir | 25 mg; max in 48 h | Exposure is the issue, not the clock |
| Need more effect | Up to 100 mg after a fair trial | A late swallow is still late |
The 50 mg default is for most patients. Several labelled groups start at 25 mg because exposure rises.
Age over 65, hepatic impairment, and creatinine clearance under 30 mL/min: consider 25 mg. Severe renal impairment roughly doubled Cmax and AUC in volunteers.
With an alpha-blocker, start 25 mg and only after the alpha-blocker dose is stable. Two vasodilators stack.
Ritonavir raises sildenafil AUC about 11-fold. The cap is 25 mg, and not more than that in 48 hours. Erythromycin or strong CYP3A4 inhibitors (ketoconazole, itraconazole, saquinavir): consider 25 mg. Saquinavir raised AUC about 210%.
Those 25 mg starts are safety cuts. They are not a claim that 50 mg was the wrong drug. Timing and food still apply at 25 mg.
A 4-hour half-life on a real evening
Both sildenafil and its active metabolite have terminal half-lives of about 4 hours. The useful window is a handful of hours, not a weekend.
A tablet taken at 18:00 is a different evening from a tablet taken at 22:00. The 4-to-5-hour response data do not make 23:30 a second peak. They mark the fade.
That is why a late change of plans wastes 50 mg more often than a 'weak batch' story. The drug left. The night did not.
Do not take a second 50 mg the same calendar day because the first one 'missed'. Maximum frequency is once per day. A missed window is a missed window.
Protein binding stays about 96% across usual concentrations. The N-desmethyl metabolite is active and shares the same 4-hour tail. The evening does not get a second hidden peak from that metabolite.
Bioavailability varies from 25% to 63% around the 41% mean. Two men can swallow 50 mg at the same hour and not share the same plasma curve. Food and CYP3A4 widen that spread further.
Vardenafil is also a short curve, half-life about 4 to 5 hours. It does not give you a longer night. The three-tablet comparison is where 36 hours versus about 4 hours is laid out.
Timing cannot lift a nitrate ban
Organic nitrates and organic nitrites in any form are contraindicated. Riociguat is the same ban. No hour-offset makes 50 mg safe next to a nitrate.
After sildenafil, the label says it is unknown when a nitrate can be given safely. Levels at 24 hours are much lower than peak. That is not a green light at 24 hours.
If chest pain starts after 50 mg, say sildenafil and the swallow time. Do not treat the tablet as an embarrassment to hide from emergency staff.
Sexual activity itself has a cardiac load. If intercourse is unsafe on cardiac grounds, 50 mg is the wrong discussion. The work-up note starts with history and risk, not with the clock.
CYP3A4 can turn 50 mg into an exposure problem
Sildenafil is cleared mainly by CYP3A4. Inhibitors raise the same 50 mg into a larger AUC. The clock does not shrink that exposure.
Ritonavir is the extreme case. AUC rose about 11-fold. The labelled cap is 25 mg, and not again inside 48 hours. A 'usual 50 mg hour' is the wrong habit there.
Erythromycin raised Cmax about 160% and AUC about 182%. Saquinavir raised Cmax about 140% and AUC about 210%. Ketoconazole and itraconazole are expected to do more than saquinavir.
Those interactions are why some men feel 50 mg as a heavy flush after a new antibiotic. The tablet did not change. The clearance did.
Grapefruit is not on the US dose table. Strong prescription inhibitors are. Bring the antiviral and antifungal list before you keep the default 50 mg.
Inducers such as some anticonvulsants can cut exposure the other way. A well-timed 50 mg then looks weak. That is a chart problem, not a meal problem.
Dose-proportional PK over 25 to 100 mg is why 100 mg is a labelled step, not a timing trick. Doubling milligrams after a late fatty meal just doubles a delayed curve.
The once-daily cap is a frequency cap. It is not a daily-ED regimen. Men who want a plateau should read tadalafil 2.5 mg, not invent daily Viagra at 50 mg.
If ritonavir is on the chart, keep the 25 mg and 48-hour cap even when the hour and the meal were perfect. Exposure, not timing, is the limiter.
Alcohol, alpha-blockers, and a wasted late tablet
In a study of sildenafil 50 mg with alcohol 0.5 g/kg, mean peak alcohol was 0.08%, and sildenafil did not add to the hypotensive effect in healthy volunteers. That is a lab sentence, not a licence to drink through the dose.
Alcohol still dulls arousal and coordination. A 'failed 50 mg' after a heavy night is often the drink, the meal, and the hour stacked together.
Alpha-blockers and other antihypertensives can drop pressure with sildenafil. The 25 mg start exists for that stack. Timing does not cancel it.
Priapism past 4 hours needs urgent care. Sudden vision loss (including NAION risk talk) and sudden hearing loss are stop-and-seek-care events. None of those wait for a second tablet.
Amlodipine 5 or 10 mg plus sildenafil 100 mg added about 8 mmHg systolic and 7 mmHg diastolic extra drop in a hypertension study. The 50 mg evening still sits on that vasodilator background if a calcium-channel blocker is already on board.
Crowded optic discs and a prior NAION episode are labelled cautions. They are not solved by taking 50 mg earlier. They are reasons to ask ophthalmology before another tablet.
Fasted versus a high-fat meal
Sildenafil tablets may be taken with or without food. That sentence is true and incomplete.
With a high-fat meal the absorption rate falls. Mean Tmax slips 60 minutes. Mean Cmax falls 29%. The hour you planned becomes two hours, and the peak is lower.
A cheeseburger-and-fries supper is the example the US patient text uses. A lighter meal is the practical workaround when the clock is tight.
An empty stomach is the cleanest way to keep the 30-to-60-minute onset people expect from 50 mg. It is not a purity rule. It is a rate rule.
The US patient leaflet names a cheeseburger and french fries as the high-fat example. That is the meal study in kitchen language. A cream sauce and fried sides sit in the same bucket.
A light plate an hour earlier is enough for most evenings. Fasting from breakfast is theatre. Skipping the late fried course is the useful move.
Tadalafil is the contrast. Cialis may be taken without regard to food. If every sildenafil night has been a heavy dinner, the failure may be the meal, not the INN. Read the tadalafil file before you abandon 50 mg.
Absolute bioavailability averages 41%, range 25% to 63%. Volume of distribution is about 105 L. Protein binding is about 96%. Those numbers explain why a delayed, smaller peak feels like a weak tablet.
The hour, the meal, the fade
Take sildenafil 50 mg about one hour before sex, fasted or after a light meal. Treat a high-fat supper as a 60-minute slip and a smaller peak.
The 4-hour half-life is why a late change of plans wastes the tablet. Do not redose the same day. Step to 25 mg or 100 mg only for labelled reasons.
The Viagra DailyMed label is the source for the hour, the fat numbers, and the half-life. A prescriber still has to see the nitrate list.
If 50 mg is well timed and still fails, the next question is the work-up, not a same-night 100 mg rescue.
